Cancellation Policy
Your scheduled appointment is a time that is reserved just for you. If you are unable to keep your appointment, please let us know 24 hours in advance so that we may schedule a new time for you. There is a $25 fee for appointments that are cancelled with less than 24-hour notice.
Prescription Refill
You call our office during normal business hours and request a refill. All calls regarding refills are transferred to a prescription refill voicemail. If you are approved for a refill, it will be sent to your pharmacy that we have on file. In the event that your refill is not approved, we will contact you to schedule an appointment. Refill requests will be processed within 48 hours. We ask that you plan accordingly.
Forms
Due to the large volume of forms that we are required to complete, our office reserves the right to charge up to $25.00 per page for the completion of forms. Dictated letters are dealt with on a case-by-case basis
What if I don’t have insurance?
Yes, we are happy to see self-pay patients. Please contact one of our offices for more information.
Is a referral required for service?
No a referral from you Primary Care Physician is not required for services.
How do I become a new patient?
Go to the home page and click new patient forms. The paperwork can be completed on your phone or tablet. Make sure to complete all the forms. Incomplete forms will not be accepted. Once completed, the intake coordinator will call you to schedule an intake appointment.
Are you accepting new patients?
Yes, all three locations are accepting new patients.
For my child’s first appointment with the psychiatrist, can a relative or friend bring them in?
We do require a parent or legal guardian to accompany the patient for anyone under the age of 18. For 18 and older, the patient is able to come by themselves.
Copays
Copays are due at the time of service. Copay payments can be made through our website at www.socialcareservices.net or by calling the office at 904-294-5329. If you have any questions about copays, please contact your insurance.
Record Requests
Please complete the Authorization of Release Form. All Record Requests take between 7-10 business days. Requests should be sent to records.scs@gmail.com,
IMPORTANCE OF MENTAL HEALTH COUNSELING:
Therapy is a relationship between people that works in part because of clearly defined rights and responsibilities held by each person. As a client in psychotherapy, you have certain rights and responsibilities that are important for you to understand. There are also legal limitations to those rights that you should be aware of. I, as your therapist, have corresponding responsibilities to you. These rights and responsibilities are described in the following sections.
Psychotherapy has both benefits and risks. Risks may include experiencing uncomfortable feelings, such as sadness, guilt, anxiety, anger, frustration, loneliness, and helplessness, because the process of psychotherapy often requires discussing the unpleasant aspects of your life. However, psychotherapy has been shown to have benefits for individuals who undertake it. Therapy often leads to a significant reduction in feelings of distress, increased satisfaction in interpersonal relationships, greater personal awareness and insight, increased skills for managing stress and resolutions to specific problems. But, there are no guarantees about what will happen. Psychotherapy requires a very active effort on your part. In order to be most successful, you will have to work on things we discuss outside of sessions.
